As Life Skills Coach for Intercept Health's Youth Quest, you will serve as a positive role model for residents to observe and learn socially effective values, attitudes and behaviors. You will contribute to the formulation and implementation of the resident transitional living plan and assist with life skills learning, transportation, and administrative duties.
RESPONSIBILITIES:
- Interviewing children and families; Conducting youth assessments. Familiarize self with youth's strengths and weaknesses in leisure skills, socialization, motor development and physical condition and plan activities to complement youth's needs. Preparing and maintaining case records. Service planning by developing individualized treatment and service plans.
- Coordinating services to minimize fragmentation of care, reduce barriers, and link children with appropriate services to ensure comprehensive, continuous access to needed medical, social, education, and other services appropriate to the needs of the resident.
- Collaborating closely with the family assessment and planning teams and other involved parties in preparation of all case plans. Preparing and carrying out social plans with children and families; Preparatory counseling with youth and families for placement or discharge, or both.
- Serve as a facilitator for young person's person-centered planning and his/her Transitional Living Plan (TLP) team. Assist youth in acquiring access to community resources (e.g., employment, housing, Chaffee funding)
- Collect and record data relevant to the youth's progress on their TLP goals and submit monthly and quarterly reports to the referring worker of the placing agency. Conduct weekly Community Life Skills Groups, review youth's progress with Life Skills Development, and discuss barriers with the Program Supervisor to determine appropriate activities and events. Initiate and supervise activities within the daily and weekly schedules that promote Life Skill Development and Independent Living.
The selected candidate will have a baccalaureate or Master's degree in social work or a field related to social work and one year of experience in providing casework services to children and families, OR; a doctor's or master's degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, special education or counseling, with a student placement in providing casework services to children and families or one year of experience in providing casework services to children and families, OR; a baccalaureate degree in any field with at least two years of experience in providing casework services to children and families.
Preference for these positions will also be given to applicants who have experience in case management experience working with youth with emotional and/or behavioral difficulties. Those who prepare these youth for living in less restrictive environments, obtaining employment or functioning in the community more effectively; and/or conducting home-based, school-based, or field based service delivery.
