Position - Wraparound Care Coordinator
Job Type: Full-time (flexible schedule 35-hour work week)
Starting Pay: $24.12 per hour (43,896.06) - $25.34 per hour (46,118.30) with yearly raises.
Wraparound Program: Wraparound is a family-centered process that employs the wraparound facilitation model to help preserve the family unit through the coordination of in home and community based services and supports and monitors risk, safety, permanency, and well-being of each family member in the home. Our program works with the whole family with children in the home up to the age of 18 who are at risk of out of home placement and have an ability to function in school, home, and community with community-based services supports.

Major Responsibilities/Activities:
- Provides home-based services to a caseload of 12 families in Erie County utilizing the Wraparound Care Coordination model
- Utilizes a strengths-based and family-centered approach to services. Meets families where they are at and fosters their growth toward their family vision.
- Partners with children, parents, guardians and service providers to create a comprehensive Plan of Care and identify any additional services the family may benefit from
- Conducts assessments throughout duration of the case
- Provides community linkages as appropriate
- Coordinates in-home, direct services for client families from Wraparound network vendors such as in-home therapists, skill builders, and parent skills workers
- Conducts a minimum of two face-to-face contacts with families each month
- Conducts Child and Family Team meetings each month
- Maintains timely and accurate documentation in two systems
- Other duties as deemed appropriate

Minimum Education and/or Professional Qualifications/Skills:
- Bachelor’s degree in a human services field and 2 years paid work experience in human
services. Experience in child welfare and/or mental health required.

Full Job Description
Title: Wraparound Care Coordinator
Department: Wraparound
Title of Immediate Supervisor: Care Coordination Director or Wraparound Supervisor (as assigned)
POSITION SUMMARY: The Care Coordinator oversees all aspects of the Wraparound process for enrolled youth and families .The Care Coordinator facilitates a planning process that builds on the youth and families’ strengths, enhances supports and promotes stability and self sufficiency. The Care Coordinator ensures that families are informed decision-making participants on well-constructed teams that formulate individualized plans to meet identified needs in the home, school and community. The Care Coordinator also assists families in developing comprehensive crisis/safety plans and accessing resources and services as needed to divert placement or hospitalization of youth. The Care Coordinator is expected to maintain a caseload of 12 children/youth and families.
RESPONSIBILITIES/DUTIES:
Casework Contacts:
• Maintain a caseload of 12 families.
• Provide direct face to face contacts with youth and families in accordance with Erie County Preventive Service Regulations
• Provide ongoing support to families in the Wraparound system including regular telephone and weekly face to face contact, and availability by
cell phone
• Conduct a safety assessment at each home visit
• Maintain a flexible work schedule, including evenings and weekends.
Child and Family Team Facilitation:
• Facilitate Child and Family meetings at a minimum of once every 30 days.
• organize meeting logistics and ensure all relevant participants are included
• Ensure the CFT’s integrate all involved systems including the family’s formal and informal support community to develop a comprehensive Plan
of Care
• utilize a health record system which allows the youth’s health information and plan of care to be accessible to the team of providers
• Assist the child and family team in integrating information on the youth and family's strengths and needs in order to develop a Plan of Care that
clearly addresses family's goals and vision
Crisis Response:
• Facilitate the development of a crisis/safety plan that promotes use of the youth and family's strengths, supports, informal and formal resources
as needed.
• Provide 24 hour on-call coverage to assist families in implementing their Crisis Plans.
Family Assessment and Plan of Care Development:
• Facilitate an assessment of the family's specific cultural identity, short- and long-term family vision, strengths, resources and needs
• Utilize trauma-informed, solution focused casework practices
• Assist in formulating a well-composed Child and Family Team.
• Refer each youth and/or family to available community resources that will assist in goal accomplishment and assist families with follow through
to linkage
• Assist the youth and family in addressing unmet needs through promoting the development and utilization of natural supports and resources,
referral and linkage to formal services
• Utilize Family Voices Network Vendor services for creative service planning to assist with unmet needs. Monitor utilization of the purchased
services for effectiveness and timeliness
• Facilitate the development of transition plans to ensure continued success of youth and families ending Wraparound Services
• Advocate obtaining needed services for all caseload clients while active in the program.
Comprehensive Transitional Care
• Follow up with hospitals/ER upon notification of youth’s admission and/or discharge to/from an ER, hospital/residential/rehabilitative setting.
• Facilitate discharge planning and follow up with hospitals/ER upon notification of a youth’s admission and/or discharge to/from ER/
hospital/residential/rehabilitative setting.
• Link youth/family with community supports to ensure that needed services are provided.
• Follow up post discharge with youth and family to ensure needed services are provided.
• Notify and consult with treating clinicians, including youth’s primary care physician, schedule timely follow up appointments, and assure that all
ordered medications are in the home and at other administering sites (e.g.schools and day care), and assist with medication reconciliation.
Collaborate with Wraparound Team:
• Collaborate with team members for case coverage and consultation
• Participate in regularly scheduled team meetings

• Maintain accurate and timely case records, including progress notes and tracking mechanisms, electronically and in each youth’s file
• Keep all youth and family information, verbal and written, confidential
Quality Improvement and fidelity to the model:
• Utilize data and outcomes to inform decision making with families
• Share family related data with the child and family team for informed decision making at team meetings
• Participate in quality improvement efforts to make changes to individual and team practices
• Evaluate efforts with supervisor and team to determine if interventions were successful, or if alternate interventions should be attempted
• Attend and participate in trainings relevant to the Children’s System of Care. A minimum of 8 training hours/year delivered through CCNY and
additional trainings as appropriate.
Agency Standards/Norms:
• Participate in regular supervision (per New Directions policy and procedure)
• Attend all mandatory in-service trainings as per NDYFS policy and ECDSS requirements
• Participate in continuing education as needed to maintain a level of expertise in the field.
• Adhere to agency and department policies and procedures

SKILL, QUALIFICATIONS, KNOWLEDGE, ABILITY & PHYSICAL REQUIREMENTS:
Minimum Education and/or Professional Qualifications/Skills
Education
• Bachelor’s degree in a human services field specializing in mental health, child welfare, juvenile justice, or other similar fields
• Two (2) years of experience working in human services in a community setting. Experience in probation, child welfare, and/or mental
health preferred
