Hello! Thank you for your interest in joining the Wellspring team as a part time Wound Care Nurse at our Transitional Care Community. TCC is a partnership between Covenant HealthCare and Wellspring to help patients transition from the hospital to home. The new facility is part of a $5 million investment by Covenant and Wellspring to provide the most advanced post-acute care and services in the Great Lakes Bay Region. The TCC also provides physical, occupational and speech therapies performed by Mary Free Bed therapists in a newly renovated, state-of-the-art facility, working closely with the nursing department to assure the highest quality in post-acute programming.
When you join our team as a part time Wound Care Nurse, you'll be part of a team of nurses, caregivers and administrative professionals who are committed to supporting our staff in their work of serving seniors and their families at our Saginaw Senior Living Campus, Transitional Care Community (TCC). In this position, you must be currently licensed by the State of Michigan Board of Nursing as a Registered Nurse and successful completion of an accredited Registered Nursing program OR as a Licensed Practical Nurse and successful completion of an accredited Licensed Practical Nursing program. Two years of nursing experience with current home health, medical, surgical, or critical care experience preferred. Wound Care experience preferred.
As a Wound Care Nurse, you will:
- Follow all established skin management protocols
- Knowledgeable in current standard of practice for treatment of pressure ulcers, non-pressure ulcers, skin tears and other skin impairments
- Ability to stage and measure wounds and document per protocol. (not only pressure but documentation on skin conditions other than wounds)
- Assess all new/readmissions for potential skin/wound concerns, within 24 hours in
conjunction with admitting nurse - Act as a resource for staff and provide on-going staff education on skin/wound care
- Complete required QA reports
- Report any issues to the DRC
- Ensure all preventative measures are implemented according to the resident’s/client’s plan of care
- Provide information for MDS
- Participate in care conferences as requested
- Provide individualized attention, which encourages each resident's ability to maintain or attain the highest level of independence and physical, mental, and psychosocial
well-being - Make weekly rounds with wound care physician
- Function as part of the IDT in coordinating care
- Receive, implement, and document new orders, regarding skin/wound care, from
physician and update information in resident’s/client’s care plan - Review resident’s/client’s care plan for appropriate goals, problem approaches and
revisions based on the resident’s nursing needs and that the Care Plan is followed - Delegate responsibilities for the direct care of specific residents regarding wound and skin care as needed
