Wound Care Medical Provider 40 hours a week
Position Summary:
Provides the full scope of internal medicine services, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance to patients.
Essential Job Functions:
Evaluate and manage patients for wound care in both outpatient and inpatient settings - including patient rounding, ensuring delivered care meets current standards of practice. Develop individualized treatment plans based on patient needs and evidence-based practices.Adheres to Healogics and BBGH clinical practice guidelines for wound careCollaborate with a multidisciplinary team including nurses, physical therapists, and other specialists to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure healing outcome goals.Ensures that education is provided to the facility staff, patients, and families regarding the management of the patient’s wound care management and prevention strategies.Document patient progress and treatment plans accurately and in a timely manner.Participate in quality improvement initiatives and contribute to the development of best practices in wound care.Maintain compliance with hospital and Healogics policies, procedures, and regulatory standards. Maintains communication and a working relationship with the facility Administrator, Medical Director, Clinical Program Director, team of health care providers, and patient family members in regard to wound care servicesDemonstrates competency in the position of wound care including assessment, debridement, wound dressings, skin substitutes and hyperbaric oxygen treatment.Participate in the Quality Improvement Program to optimize quality service and maintain a high level of patient satisfaction.Serves as a patient advocate, initiating action to provide continuity of care outside the facility setting.Follows HIPAA regulations and ensures that the confidentiality of patients’ medical, personal, and financial records is maintained.

- Required: Graduate from an MD, DO, PA or NP program and Credentialed by BBGH in Wound Care
- Experience: Minimum of one year of recent experience in an acute care setting.
- Licensure/Certifications: Per contract - located in on a credentialing file maintained by Executive Assistant and Certification in wound care (e.g., CWSP, CWS, CWOCN) is preferred. and BLS for Healthcare Providers.
