About the Job
- Develops goals and objectives for the Annual Plan of Program Operations and ensures implementation in accordance with the requirements of DC Health, Community Health Administration, WIC State Agency, in collaboration with Program and Site Nutritionist and Breastfeeding Coordinator(s) as appropriate.
- Ensures that health and nutritional assessments of WIC applicants are completed daily to determine medical/nutritional risk, including assessment of medical history, health data, hematological, dietary measures and breastfeeding goals;
- Plans and coordinates outreach and other program activities with COH clinic staff, health care providers, and community-based organizations to identify potential WIC participants; Ensures that outreach activities are directed at the highest priority groups, such as pregnant and breastfeeding women, and infants less than 12 months of age, in order to meet performance standards.
- Develops and/or approves nutrition education and breastfeeding activities, lesson plans materials, visual aids, etc., and ensures that State Agency approvals of such materials are secured, as required
- Develops and implements policies and procedures to ensure that documentation of services and accountability of property are maintained as required by the State Agency and Federal rules/guidelines
- Plans and conducts special initiatives according to the project work plan directed at assuring that performance standards are reached or surpassed in the area of program utilization, priorities served, nutrition services and other key performance areas and measures progress quarterly.
- Maintains liaison with the WIC State Agency and attends monthly meetings for local implementing agencies, quarterly nutrition education training, and other local and national WIC meetings.
- Meets with supervisees regularly and provides real-time feedback, training, support, and coaching as needed. Creates meeting agendas, sends meeting agendas in advance when possible, and documents concerns promptly as needed.
- Applies principles learned through diversity, equity, and inclusion (DEI) trainings and/or trauma informed care trainings (TIC) and leads with a DEI and TIC lens when appropriate.
- Reviews the performance of team members in a timely manner, including completing 90 day and annual performance evaluations per their due dates, documenting any issues or changes, providing recommendations for professional development and/or training opportunities, and recognizing staff when appropriate.
- Monitors HANDS database daily to ensure that notes, assessments, and data related to family health is entered appropriately. Provides regular feedback to supervisees on the administrative aspects of their work.
- Ensures that the local program adheres to all Federal and State regulations and guidelines. Ensures accurate and timely submission of required reports to DC Health.
- Oversees proper certification of eligible WIC applicants.
- Ensures that procedures are in place for individuals to receive or are provided referrals to health services, breastfeeding and other community health and nutrition programs as needed to address social and health needs of applicants.
- Directs and coordinates a referral system with health care providers and other community health and nutrition programs.
- Directs all aspects of the local program component of the food delivery system including distribution of the WIC benefits to participants and assurance of safeguards for WIC benefits.
- Develops criteria for evaluation at the start of each grant cycle, conducts evaluation quarterly, and assesses progress on a continual basis.
- Documents any relevant patient encounters, including face to face, telephone, and electronic communications, in eCW within 3 business days.
- Completes other duties as assigned.
Minimum Qualifications:
- Master's degree in clinical nutrition, nutrition science, dietetics or nutrition-related field required.
- Current license as a dietitian or nutritionist (LD, LN) in the District of Columbia required.
- Three years of experience in planning and program development within a health or community nutrition program including supervising personnel.
- Non-supervisory professional level experience in a WIC Program may be substituted for up to two years.
- Proof of required vaccinations is required. This includes, but may not be limited to, Flu and Covid. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.
- Experience providing culturally competent, patient-centered education in a low-income setting highly required.
- Ability to multi-task and problem solve required.
- Ability to work in a fast-paced, team-based setting required.
- Ability to work flexible hours (i.e. evenings, weekends) to provide increased access to patients required.
- Ability to travel to other sites for the purposes of training, meetings, and providing cross-coverage required.
- Experience in community health center setting preferred.
- All Health Staff must receive the required vaccinations. This includes annual flu and covid vaccines. Medical or religious exemptions may be requested.
