Key Responsibilities:

Conduct health screenings and assessments

Develop and implement individualized wellness plans

Educate clients on health management and disease prevention

Collaborate with healthcare providers and community resources

Maintain accurate health records and documentation

Qualifications:

Registered Nurse (RN) license in Illinois

Experience in community health or wellness programs preferred

Strong communication and interpersonal skills
