The Role:
The Wellness Coordinatoris responsible for overseeing and coordinating the comprehensive applicable care management of patients on a 360° level. This role serves as a key point of contact for patients with applicable conditions, working closely with healthcare providers to optimize their care, enhance patient outcomes, and improve overall quality of life. The Wellness Coordinator plays a vital role in coordinating and implementing personalized care plans, providing ongoing support, and facilitating effective communication between patients, caregivers, providers, and other healthcare professionals.
Positional responsibilities include, but are not limited to:
- Company representative housed in a senior living community providing support to residents, staff, families, and providers.
- Take monthly vitals and weights on community residents.
- Support in the care and coordination and communication of services for residents.
- Support residents with telehealth appointments and technology.
- Provide daily wellness checks on company program enrolled programs.
- Provide companionship to community residents.
- Participate in community and company events.
- Communicate critical findings to appropriate contacts, as needed.
- Provide education and support to patients and their families on managing their applicable conditions, including self-care techniques, symptom management, and resources available.
- Facilitate effective communication among patients, healthcare providers, and other members of the care team to ensure seamless coordination of care.
- Participate in quality improvement initiatives, collaborating with the healthcare team to enhance patient quality of life.
- Assists lead or supervisor in orienting, training, and mentoring staff.
- Provides ongoing training to staff as needed.
- Additional responsibilities, as assigned.
Requirements:
- At least 18 years of age
- High school diploma, GED, or equivalent
- Medical Assistant Diploma required
- Education or experience in patient care
- Education or experience in case management
- Possess outgoing, professional, and self-initiative qualities
- Ability to work independently
- Ability to stand for long periods of time
- Willingness to travel to assigned location daily
- Willingness to attend/ work community events, which may be outside the typical M-F, 8-5 pm schedule
- Phlebotomist License (Preferred)
