Position Summary:
Under general supervision, the Team Lead is responsible for completing comprehensive intakes and assessments with individuals diagnosed with mental health (MH), substance abuse (SA), or co-occurring disorders. This includes collaborating with families, other supports, and collateral agencies to determine individual needs, strengths, social and medical history, and overall functioning levels.
Key responsibilities include developing treatment plans and conducting regular reviews, providing individual, group, and family counseling, and liaising with MH and Probate Courts as required. The Team Lead will also play an essential role in crisis intervention, supporting individuals at risk of harming themselves, others, or property by addressing immediate concerns and developing alternative service links to prevent out-of-home placement or hospitalization.
This position involves participation in training, conferences, and workshops to maintain up-to-date continuing education and core hours. The Team Lead will be available 24/7 to provide services through telephone or face-to-face interventions in various settings.
Key Responsibilities:
- Conduct comprehensive intake and assessments with MH/SA or co-occurring consumers, families, and collateral agencies to identify individual needs, strengths, and social/medical history.
- Develop, implement, and review individualized treatment and recovery/resiliency plans based on consumer-identified goals.
- Provide individual, group, and family counseling as needed to support consumers.
- Serve as a liaison with Mental Health Court and Probate Court as required.
- Engage in crisis intervention and support consumers at risk of self-harm, harm to others, or property damage by addressing immediate concerns and developing alternative service links.
- Prevent out-of-home placement or hospitalization by addressing crises and stabilizing consumers in their current environment.
- Maintain up-to-date core and continuing education hours by attending relevant training, conferences, and workshops.
- Be available 24/7 to provide support via telephone or face-to-face interventions in any setting.
Qualifications:
- Must hold licensure as one of the following by the Georgia Composite Board of Professional Counselors, Social Workers, and Marriage and Family Therapists:
- Licensed Clinical Social Worker (LCSW)
- Licensed Professional Counselor (LPC)
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Associate Professional Counselor (LAPC) under an approved clinical supervision plan.
- Graduate Interns from accredited universities for Social Work or Counseling are also encouraged to apply.
- At least one year of experience in Intensive Family Intervention (IFI) or Substance Abuse (SA) is a plus.
- Strong knowledge of community resources and the ability to connect consumers with appropriate services.
- Excellent communication skills and the ability to collaborate effectively with diverse populations.
- An understanding of cultural diversity and its impact on the recovery process.
