Job Details
- Provide comprehensive care for patients with acute and chronic illnesses or injuries in our urgent care setting.
- Collaborate with medical assistants, nursing, other mid-level providers, physicians, and care coordinators to ensure high-quality patient care.
- Conduct assessments, diagnoses, treatment, management, education, and health promotion for patients and their families.
- Perform minor surgical procedures like incision and drainage, laceration repair, and suture removal.
- Support transitions in care, including hospital and ER follow-ups.
- Participate in care coordination and contribute to the Primary Care Medical Home model.
- Ensure accurate and timely clinical documentation in compliance with organizational standards.
Skills You Need
- Accredited nursing or physician assistant degree with completed nurse practitioner program.
- Valid State of Connecticut licensure, with DEA and state narcotic registration eligibility.
- Experience in urgent care or ambulatory settings preferred.
- Strong communication and collaboration skills to work effectively with a diverse team and patient population.
