We are seeking a dedicated and experienced Urgent Care Clinician to join a community-focused health organization committed to delivering quality care to diverse populations. Type: Full-Time/ Perm
Schedule: 8:30am – 5:00pm (Some days open until 7:00pm)
Location: Connecticut
Duties and Responsibilities:
Deliver urgent care services in alignment with the clinician's medical specialty, ensuring comprehensive care for patients.
Develop individualized care plans that include taking medical histories, conducting physical examinations, diagnosing, and coordinating appropriate treatments or referrals.
Utilize resources effectively for diagnostics, including lab work, to support efficient and cost-effective patient care.
Provide primary care interventions as appropriate based on the patient's primary concerns and any existing health conditions, while coordinating care within the primary care team.
Communicate effectively with colleagues, patients' families, and support staff to ensure cohesive care and patient understanding.
Refer cases requiring specialized services and collaborate with the primary care team to maintain continuity of care.
Maintain accurate patient records and ensure follow-up, providing quality oversight through charting and documentation.
Actively participate in staff meetings, quality improvement programs, clinical task forces, and continuing education initiatives.
Undertake additional responsibilities as needed to support organizational goals in quality care and patient satisfaction.
Required Skills:
Degree from an accredited medical school (MD or DO) or an accredited program for Advanced Practice Registered Nurses, Doctor of Nursing Practice, or Physician Assistants.
Completion of an accredited residency in a relevant specialty (for physicians).
Active medical license for clinical practice.
Certified in Basic Life Support.
Ability to collaborate effectively and compassionately with a diverse patient population and healthcare team.
Desired Skills:
Bilingual proficiency in Spanish and English is a plus.
Experience working in ambulatory or outpatient settings.
Strong interpersonal skills for effective patient and team interactions.
