We are looking to hire several BSN Prepared RNs with 1+ year of nursing experience in community health nursing or home care nursing to work within the scope of a UAS Assessment RN.
Chose to work in ONE borough of your choice - we have openings in Manhattan OR Brooklyn OR Queens OR Staten Island OR Bronx NY
In this full time role, the UAS Assessment RN will independently perform home visits to conduct functional, psychosocial and environmental safety assessments, determine eligibility of enrollees, and establish an individual plan of care that enables them to remain safely at home despite long-term care needs.
Company rewards full time Assessment Nurses with a higher reimbursement rate when conducting extra visits.
RN must have working knowledge of Medicare, Medicaid and NYS DOH standards and regulations
Bilingual English/Spanish, Mandarin, Cantonese, Korean, Hindu, and Arabic a PLUS but not required
Work schedule is Monday thruy Friday 830 am to 5 pm
