Good Samaritan Hospice is seeking a qualified LPN Triage Nurse FT for the Roanoke service area.

The Triage LPN is responsible for taking calls from patients and families, assess the patient's situation and provide instructions for symptom management or medical treatment. The triage nurse consults with the RN Case Manager if there is a need for a visit.

The LPN will support our hospice field staff by making phone calls to physicians, patients, families, and ordering medications and/or medical supplies.

The LPN will support the Director and staff by prepping for interdisciplinary team meetings, reviewing referral paperwork for eligibility, coordinating and scheduling, and keeping the team running smoothly. The triage nurse will attend and scribe team meetings upon request.

This is an In-Office position.

The schedule will be Monday- Friday, 8:30 AM- 5:00 PM.
