About The Position
Job Title: Registered Nurse Job Specialty: Hospice Job Duration: 13 weeks Shift: Day - 5x8 Hour (08:00 - 17:00) Guaranteed Hours: None Experience: Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience, depending on specialty License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and OASIS, recent specialty experience, reliable transportation, driver's license, auto insurance Job Description: - Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Travel to home visits and periodic office meetings; must be willing to travel up to 75 miles from the assigned clinic. - Manage a caseload that averages 5-8 patients per day, with productivity expectations of 30 points per week. - Types of cases include Start of Care, Wound Care, and Follow-up Visits. - Be available for weekend and on-call rotations as required. - Complete all patient documentation within 24 hours of the appointment. - Attend weekly meetings at the clinic and float to nearby clinics if necessary to meet financial needs. - Ensure proficiency in HomeCare HomeBase and OASIS systems within the last 6 months or complete a retraining course during compliance. - Dress code requires scrubs.
