About The Position
**Job Title:** Registered Nurse **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** None **Experience:** Minimum 1 full year in Adult Home Health or Hospice experience **License:** Must have license in hand **Certifications:** CPR, BLS **Must-Have:** Proficiency in OASIS and HomeCare HomeBase used within the last 6 months or completion of a retraining course **Job Description:** - Provide, coordinate, and direct home nursing care according to physician's orders based on agency policies and procedures. - Responsible to the Clinical Manager and directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Maintain reliable transportation, a current driver's license, and agency-required liability insurance. - Average caseload of 5–8 patients per day. - Handle Start of Care, Wound Care, Follow-up Visits, and other case types. - Willing to travel up to 75 miles from the home clinic; mileage reimbursed at the IRS standard rate. - Participate in on-call rotations 1-2 times per month. - Complete all documentation within 24 hours of the patient appointment. - Accept every patient scheduled regardless of distance; no guaranteed hours, and productivity must meet 30 points per week. - Open to traveling to other nearby clinics if needed to pick up patients. - Meet weekly productivity expectations of 25-30 units. - Work under a dress code requiring scrubs. - Utilize EMR systems proficiently. - Participate in appropriate training or retraining if necessary.
