Job Title: Registered Nurse - Home Health Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Active RN license Certifications: CPR, BLS Must-Have: Proficiency in OASIS and HomeCare HomeBase (HCHB) EMR systems Job Description: - Provide, coordinate, and direct home nursing care based on physician orders and agency policies - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses - Conduct home visits and periodic office meetings - Travel up to 75 miles from home clinic - Complete all documentation within 24 hours of the patient appointment - Accept all patients on the schedule, regardless of distance between appointments - Hit weekly productivity points equivalent to 40 hours per week - No guaranteed hours, must meet productivity points - Proficiency in Oasis and HomeCare HomeBase EMR systems - Minimum 1 year of relevant experience required - Must have reliable transportation, driver's license, and agency-required liability insurance.
