About The Position
**Job Title:** Registered Nurse **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** None **Experience:** Minimum 1 full year in Adult Home Health experience or Hospice experience **License:** Must have current license in hand **Certifications:** CPR, BLS **Must-Have:** - Proficiency in OASIS and HomeCare HomeBase (HCHB) EMR systems, used within the last 6 months, or completion of a retraining course during compliance - Reliable transportation, current driver’s license, and agency-required liability insurance - Willingness to travel up to 75 miles from the home clinic **Job Description:** - Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. - Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Average number of patients per day: 5-8, with a points-based productivity system (30 points per week). - Types of cases include Start of Care, Wound Care, and Follow-up Visits. - Participate in on-call rotations, including two weekend days and two weekdays per month. - Complete all patient documentation within 24 hours of appointment. **Additional Notes:** - Must meet weekly productivity points of 30, equivalent to 40 hours of work per week. - In cases of low patient census, potential floating to nearby clinics for additional hours.
