About The Position
Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year Hospice experience License: Must have a valid nurse license Certifications: CPR, BLS Must-Have: Proficiency in OASIS and HomeCare HomeBase (HCHB) Job Description: - Provide, coordinate, and direct the provision of home nursing care according to physician's orders based on agency policies and procedures. - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Drive to patient homes daily and attend clinic meetings. Must have reliable transportation, a valid driver's license, and liability insurance. - Responsible for an average of 5-8 patients per day, achieving roughly 6-8 productivity points. - Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. - Complete all documentation within 24 hours of the appointment with the patient. - Willingness to travel up to 75 miles from the home clinic, with mileage reimbursed at the standard IRS rate. - Local candidates accepted at the same rate, must have a license in hand. - Open to call rotations and potential for floating to nearby clinics if necessary. - Dress code: Scrubs - Efficient use of EMR system required, specifically HomeCare HomeBase. - Responsible to the Clinical Manager, must adhere to all agency protocols and procedures.
