About The Position
**Job Title:** Registered Nurse Case Manager **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** Not guaranteed **Experience:** Minimum 1 full year of Adult Home Health or Hospice experience **License:** Must have a license in hand **Certifications:** CPR, BLS **Must-Have:** Proficiency in OASIS and HomeCare HomeBase, reliable transportation, driver's license, and auto insurance **Job Description:** - Provide, coordinate, and direct home nursing care according to physician's orders based on agency policies and procedures. - Utilize the nursing process competently. - Responsible to the Clinical Manager. - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Assigned between 5-10 patients per day, amounting to approximately 6-8 points. - Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. - Must be willing to travel up to 75 miles from the home clinic; mileage reimbursed at the IRS standard rate. - Perform weekly rotation, possibly including weekends and on-call duties. - Proficient in completing all documentation within 24 hours of patient appointments. - Meet weekly productivity expectations of 30 points, which equates to roughly 40 hours per week. - Opportunity to float to nearby clinics if patient census is low to ensure adequate financial compensation. - Dress code: Scrubs. - Expected to travel approximately 150-200 miles per week. - Must have recent experience with OASIS and HomeCare HomeBase software.
