About The Position
**Job Title:** Registered Nurse I **Job Specialty:** Home Health **Job Duration:** 13 Weeks **Shift:** Day 5x8-Hour (08:00 - 17:00), Monday-Friday **Guaranteed Hours:** No **Experience:** Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience **License:** Must have license in hand, no exceptions **Certifications:** CPR, BLS **Must-Have:** - Proficiency in HomeCare HomeBase (HCHB) and Oasis - Reliable transportation, driver's license, auto insurance - Willingness to travel up to 75 miles from the home clinic - Ability to complete documentation within 24 hours - Open to on-call rotations **Job Description:** - Provide, coordinate, and direct home nursing care according to physicians' orders - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses - Manage a patient load of 5-10 patients per day, equating to roughly 6-8 points - Conduct various types of cases including Start of Care, Wound Care, Follow-up Visits, etc. - Engage in home visits and periodic office meetings - Drive responsibly to patients' homes daily for care delivery - Attend weekly meetings at the clinic - Manage travel and documentation tasks with point values contributing to productivity expectations of 30 points per week - Adapt to weekend and on-call requirements every 4 weeks - Work effectively to hit weekly productivity targets based on assigned patients - Be open to floating to nearby clinics if patient census drops - Maintain patient privacy and follow agency policies and procedures - Receive mileage reimbursement from patient home to patient home
