About The Position
Job Title: Registered Nurse - Home Health Job Specialty: Home Health Nursing Job Duration: 13 weeks Shift: Day, 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No guaranteed hours, based on productivity points system Experience: Minimum 1 year of Adult Home Health or Hospice experience License: Active Registered Nurse license Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase (HCHB) and Oasis EMR - Reliable transportation, driver's license, and auto insurance - Comfortable with home visits and up to 75 miles of travel - Ability to complete documentation within 24 hours - Willingness to accept and see all scheduled patients - Open to on-call rotations if required Job Description: - Provide, coordinate, and direct home nursing care according to physician's orders - Use the nursing process based on agency policies - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses - Perform start of care, wound care, follow-up visits, and other home health duties - Proficiency in using HomeCare HomeBase (HCHB) and Oasis EMR - Must hit weekly productivity points equating to 40 hours per week - No guaranteed hours, contractors may need to float to nearby clinics for additional hours when needed - Open to traveling up to 75 miles from the assigned care center - Must complete all documentation within 24 hours of the appointment - Accept every patient on the schedule and travel between patients as required - Proficiency in Oasis EMR and recent Home Health experience mandatory - Compliance with agency policies, procedures, and standards of practice - Participate in periodic office meetings and trainings as required
