Location: Bronx, NY, 10457
Job Type: Travel Contract (13 Weeks)
Shift: Monday to Friday, 8:00 AM – 4:30 PM (No weekends or holidays)
Compensation: $66/hour

Key Responsibilities:

• Perform case management, including discharge planning and utilization review.
• Coordinate referrals to nursing homes, home care services, and other facilities.
• Collaborate with interdisciplinary teams to ensure effective patient care transitions.
• Utilize CAREPORT, MCG, Interqual, and Allscripts effectively.

Minimum Requirements:

• Education: BSN required.
• Certifications: PRI and Screen certifications required.
• Experience:
  • Strong background in case management and utilization review.
  • Experience working on units such as family medicine, stepdown/med-surg, and pulmonary.
  • Positive attitude and ability to handle diverse caseloads.
