**Job Title:** Registered Nurse (RN) **Job Specialty:** Case Management **Job Duration:** 13 weeks **Shift:** Monday to Friday, 8:00 AM - 5:00 PM **Guaranteed Hours:** Not specified **Experience:** Minimum 1 year of case management experience **License:** Registered Nurse (RN) License **Certifications:** None specified **Must-Have:** - Travel experience - Case Management experience **Job Description:** - Coordinate patient care across multiple departments to ensure effective discharge planning and care transitions. - Work closely with a multidisciplinary team of 7 RN Case Managers, 5 Social Workers, and 1 Case Management Assistant. - Manage patient caseloads with a ratio of RN: 1:25, SW: 1:40. - Utilize various electronic medical record systems, including Midas, Meditech, Interqual, and Care Port. - Exhibit strong communication skills and extensive clinical knowledge. - Adhere to dress code: Navy scrubs for RNs, Olive green top and black bottoms for Social Workers and Case Management Assistants, and Business Casual with a White Lab Coat where required. - Be open to occasional weekend work based on operational needs. - Work in an environment equipped with Work Stations on Wheels (WOWs) for efficient mobility and access to patient data. - Collaborate effectively with patients, families, and healthcare teams to implement and evaluate comprehensive care plans. This position is within an established and supportive team known for its performance with new equipment and technology, offering a friendly and professional work environment. Candidates with HCA Case Management Experience and familiarity with the Midas software system are preferred.
