**Job Title:** Care Manager **Job Specialty:** Clinical-Nursing **Job Duration:** 13 Weeks **Shift:** Days 5x8 **Guaranteed Hours:** 40 hours per week **Experience:** Minimum two (2) years of health care experience as a Registered Nurse **License:** Licensed to practice as a Registered Nurse in the state of North Carolina **Certifications:** Not specified **Must-Have:** Strong assessment and critical thinking skills **Job Description:** - Provide comprehensive support and expertise through assessment, planning, implementation, and evaluation of individual patient needs. - Enhance patient management and satisfaction, promote continuity of care, and ensure cost-effectiveness through case management, utilization review, and discharge planning integration. - Identify and prioritize patient cases daily, and ensure proper documentation and care planning. - Engage in daily care management touchpoints and attend CAPP meetings to discuss patient progression and discharge planning. - Modify and adjust discharge plans based on new information and meetings. - Attend weekly Complex Care Meetings, present patient cases, and collaborate to solve complex issues. - Proactively identify high-risk cases and coordinate family meetings as needed. - Educate patients, families, and care teams about available community resources and services. - Participate actively in care progression and transition planning, ensuring barriers to discharge are addressed. - Monitor observation patients and continuously assess discharge readiness. - Facilitate medication resource management and verify the patient's understanding and agreement with the discharge plan. - Consult with social workers and utilization managers as per departmental protocols. - Refer administrative tasks to Care Management Assistants as necessary.
