**Job Title:** Care Manager **Profession:** Manager **Specialty:** Care Management **Duration:** 13 weeks **Shift:** Days **Hours per Shift:** 8 **Experience:** Minimum of 2 years of healthcare experience as a Registered Nurse **License:** Licensed to practice as a Registered Nurse in North Carolina **Certifications:** N/A **Must-Have:** Strong assessment and critical thinking skills **Description:** The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness through the integration and functions of case management, utilization review, and discharge planning. The Care Manager must be a highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies. **Essential Duties:** - Identify and prioritize cases by reviewing work lists to identify new admissions. - Conduct and document assessments and a plan of care in the electronic health record per departmental guidelines. - Participate in daily care management meetings according to established protocols. - Communicate with the Care Management Assistant to share priorities, if indicated. - Attend weekly meetings to collaborate on patient care plans and update as needed. - Coordinate family meetings as necessary to support care progression. - Provide education on community resources and facilitate referrals to post-acute providers as necessary. - Monitor the progression of care and identify readiness for discharge in collaboration with the care team. - Verify patient understanding and agreement with the discharge plan. - Refer administrative tasks to the Care Management Assistant as required. - Collaborate with Social Workers and Utilization Managers according to departmental protocol.
