Job Details
- Documented proof of two MMR vaccines or Positive IGG Titer. No declinations accepted.
- Required for all travelers. Preferred to be completed prior to start, can be completed onsite. PAPR will not be completed on site. WI and MI require 3M 1860 Regular, 3M 1860 Small or 3M 1870+. Updated annually.
- Documentation of vaccination series completion OR documentation of declination to vaccine to be signed and dated by traveler.
- Flu season is October 1, 2024 through March 31, 2025. Exemptions/Declinations are accepted, but candidate will be required to wear a mask at all times.
- 1st PPD within 1 year, 2nd PPD within 30 days of start. And/or Negative IGRA within 90 days of start date lab printout prior to start. TB Questionnaire must be updated annually. If positive TB record; Requires Proof of Chest X-Ray within last 1 year to be accompanied by TB questionnaire.
- Documented proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles) by a healthcare provider. If negative, provide proof of two doses of Varicella vaccine, four weeks apart. No declinations accepted.
- Valid 10 years from the administered date. No declinations or attestations accepted.
- Statement of good health or completed physical exam prior to first assignment, updated annually thereafter.

- Qualifying reference(s) completed prior to first assignment. At least two professional references
- Include Social Security Trace, Employment Verification, OIG Search and National Sex Offender. Completion of: 7 year search for-SSN Trace, County Resided and Employed search, National Criminal, OFAC, and VSOP completed within 30 days of first assignment. Updated every 1 year thereafter or if break in service > 90 days, must run counties listed during break in service.
- Run within 90 days of start. Reran if break in service >90 days.
- Clinical only, to be completed within one year of start; updated annually for specialty.
- Within 30 days prior to the start of the first assignment with the client, updated annually thereafter. If break in service > 90 days, retesting is required. 12 panel UDS: 1) Amphetamine (AMP) 2) Cocaine (COC) 3) Marijuana (THC) 4) Phencyclidine (PCP) 5) Methamphetamine (mAMP or MET) 6) Barbiturates (BAR) 7) Benzodiazepines (BZO) 8) Buprenorphine (BUP) 9) Methadone (MTD) 10) Oxycodone (OXY) 11) Morphine (MOP) 12) Ecstasy (MDMA). Must show proof of all 12 drugs tested, if some are tested within sub-category, must provide itemized list from lab. Include MRO Review for prescriptions or negative-dilute.
- Forward facing headshot with a white or blue background. No alterations or filters.
- Verification of education relevant to candidate’s clinical work. Must be completed within 90 days of start. If break in service > 90 days, must rerun full report. If education verification fails, provide official transcripts with failed attempt. Diplomas not accepted.
- Provide upon initial assignment and update upon each extension. Must be on agency letterhead. Start and End dates must reflect what is within the technology.
- At the completion of assignment, vendor partner sends survey link to contract worker to be completed within 72 hours. Survey details can be found in the document library.
- Third-party employment verification for the past seven years. If third party employment verification fails, provide proof of attempts and internal verification on agency letterhead. Must match resume/profile. Updated every 1 year thereafter. If break in service > 90 days, must rerun full report.
