Travel PT Hospital Setting - Connected Health Care, LLC

Connected Health Care is seeking a dedicated and experienced Contract/Travel Physical Therapist to join our client's inclusive and friendly team. This contract Travel PT - Hospital Setting role is based in Cumberland and offers a contract rate of $65.00 per hour.

As a Travel PT in a mixed setting, you will have the opportunity to work in various environments, including inpatient acute care, swing bed, outpatient, and skilled nursing facility (SNF), focusing on outpatient care.

In your first few weeks in this Physical Therapist role, you can expect to:

• Evaluate, plan, and administer comprehensive physical therapy modalities/regimens to patients in various settings
• Develop and implement individualized treatment plans based on patient needs and goals
• Collaborate closely with in-house providers to ensure coordinated and effective patient care
• Maintain accurate and up-to-date patient records.

To apply for this physical therapist job, you will need a valid Physical Therapist license in the state of Wisconsin. You will also require the following:

• Experience in a mixed setting, particularly outpatient care
• Clinical and interpersonal skills
