**Certifications:** CPR and BLS
**Must-Have:**
- Recent experience in the home health or hospice specialty
- Proficiency in HomeCare HomeBase (HCHB) and OASIS
- Reliable transportation with a current driver's license and agency-required liability insurance
- Willingness to travel up to 75 miles from the assigned care center
- Open to being on-call depending on location
**Job Description:**
- Provide, coordinate, and direct the provision of home nursing care according to physician's orders and agency policies and procedures through the competent application of the nursing process.
- Responsible to the Clinical Manager
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Conduct home visits and attend periodic office meetings as required
- Maintain an average of 5-10 patients per day, translating to roughly 6-8 points, with 30 points expected per week
- Handle various case types, including Start of Care, Wound Care, and Follow-up Visits
- Dress code required: Scrubs
- Must travel approximately 50-75 miles per week
- Use of electronic medical record (EMR) systems: HomeCare HomeBase (HCHB)
