- Proficiency in HomeCare HomeBase (HCHB) and Oasis
- Reliable transportation with current driver's license and liability insurance
- Open to traveling up to 75 miles from home clinic
- Comfortable with 30 points per week productivity expectations
Job Description:
- Provide, coordinate, and direct home nursing care based on physician's orders and agency policies
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Conduct home visits and periodic office meetings
- Average 5-8 patients per day with various cases such as Start of Care, Wound Care, and Follow up Visits
- Complete all documentation within 24 hours of the appointment
- No guaranteed hours, must hit weekly productivity points
- Ability to float to nearby clinics if needed for patient coverage
- Proficiency in HomeCare HomeBase and Oasis EMR systems
- Conduct all visits and activities according to agency standards and guidelines
