Travel Physical Therapist (PT) - $2,186 per week in Dixon, IL at Infojini Healthcare

ID: 52153505 Shift: 8H Days Monday – Friday 8:00-4:30 Description: Unit Notes:**ALL PAPER WORK DUE THURSDAY at NOON BEFORE START DATE**- # of patients treated per day: 10/day - 1:1 treatments. Treatments are scheduled for 45mins each and Evals are 1Hr each- General Certification Requirements (BLS, etc.): BLS- Other Certs Required for all PT: State licensure- Common diagnosis / Types of patients: Outpatient population - primarily adults-geriatrics & adults-geriatrics could include, but not limited to: Fall risk, neuro, general medical recovery, unsteady gait, general weakness, ortho (primarily ortho). Some peds (>9 y/o with ortho diagnoses), - Acute Inpatient, Acute Rehab, Skilled, Outpatient?: Outpatient- What areas are they required to cover?: Outpatient- Experience or 'Must have' skills & common job duties: Strong customer service skills & communication skills. Flexibility with a positive attitude. No new grads. Need 1 year of experience with approximately 1/2 of that being outpatient experience.- EMR: Cerner Community Works for inpatient & outpatient charting- PT Manager/Supervisor: Lori Jarrett - director of rehab services- PTAs?: Utilize PTAs for treatments. Therapist sees patient at least every 6th visit.- Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?): Prefer 4 - 10Hr shifts. Or 8Hr shifts/5 days per week (9a-530p or 10a-630p)- Weekend Requirements: No weekends- Holiday Requirements: No holidays- Scrub Color/Dress Code: Black, navy, or royal blue scrubs or business casual- Department specific orientation: 1st day - going over procedures/education, etc & observing; 2nd day - orienting, may get assigned to 1 or 2 treatments/evaluations; days 3-5 - assigned treatments/evaluations with extra time for documentation.
