Job Details:

• Pay: $ 2680 per week

• Physical Therapist (PT)- Home Health

• Duration: 13-weeks

• Shift Info: Monday-Friday and one weekend a month rotation

• Guaranteed Hours: 40 hours per week

• Positions Open: 1

• Required to prioritize start of care visits

• Required to supervise Physical Therapy Assistants

• Next day mandatory reporting of start of care visits

• Clinical notes and other documentation due within 48 hours of visit

• Maintain timely communication with supervisor, intake, and office

• Be generally responsive during normal business hours

• Documentation System: Kinnser

• Equipment Issued: iPad, Verizon One Talk

• Parking and Toll Reimbursement: Paid

• Company Car: Option

Requirements:

• Active Unrestricted CA License 

• Active BLS via American Heart Association

• Must have 1 year of Home Health experience

• Must be able to provide at least 1 supervisory references
