License: Valid state pharmacy license required
Certifications:
- Required: BLS, PharmD, State Licensed Pharmacist
- Preferred: BCPS (Board Certified Pharmacotherapy Specialist)
Must-Have:
- Experience with Pyxis systems
- COVID-19 vaccine and booster are required (no exemptions)
- Flu vaccine required (no exemptions)
- EPIC experience required
Job Description:
- Provide pharmaceutical care and medication management for patients in an inpatient setting.
- Work on anticoagulants, pharmacokinetic dosing, and renal dosing.
- Perform therapeutic medication monitoring, including warfarin dosing.
- Manage medications for conditions including, but not limited to, bone and joint, cardiovascular, dermatologic, endocrine, gastrointestinal, genitourinary, hematologic, immunologic, infectious diseases, neurologic, oncologic, psychiatric, pulmonary, renal, and rheumatologic.
- Collaborate with the healthcare team to ensure optimal patient outcomes.
- Complete necessary modules pre-start and annually.
