About The Position
Job Title: Registered Nurse Job Specialty: Hospice Job Duration: 13 weeks Shift: Day, 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year Hospice experience License: Must have a valid nursing license; cannot apply with a pending license Certifications: CPR, BLS Must-Have: - Proficiency in OASIS - Familiarity with HomeCare HomeBase EMR system Job Description: - Provides, coordinates, and directs the provision of home nursing care according to physicians' orders, based on agency policies and procedures through the competent application of the nursing process. - Responsible to the Clinical Manager, directly supervises, and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conducts home visits and periodic office meetings. Reliable transportation, a current driver's license, and liability insurance are required. - Expected to manage an average of 5-8 patients per day, translating to approximately 6-8 points per day and a weekly productivity expectation of 25 points. - Responsible for types of cases including Start of Care, Wound Care, and Follow-up Visits. - Interaction with patients on weekdays with no weekend rotation or on-call duties, unless otherwise discussed. - Will cover territories including Dorchester, West Roxbury, and Jamaica Plain within a mileage radius of up to 75 miles; mileage reimbursed at the IRS standard rate from one patient's home to the next. - Emphasizes the importance of documenting all patient information within 24 hours of the appointment to ensure proper billing and follow-up care. - Open to floating to nearby clinics to pick up additional patients if assigned patient load is insufficient. - Must accept every patient on their schedule and complete all assignments as required. Note: This position is suited for a local applicant who can meet the commuting requirements or a traveler willing to meet the travel expectations. Ensure possession of a reliable vehicle, a valid driver's license, and auto insurance.
