About The Position
Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase (HCHB) and OASIS Job Description: - Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. - Responsible to the Clinical Manager. - Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Perform home visits and attend periodic office meetings. - Travel up to 75 miles from the home clinic; must have reliable transportation, a current driver's license, and agency-required liability insurance. - Handle an average of 5-8 patients per day, with productivity points totaling 30 per week. - Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. - Participate in weekend rotation and on-call duties twice a month. - Cover designated territories, including specific areas for patient visits. - Utilize the HCHB EMR system for charting and documentation. - Complete all documentation within 24 hours of the appointment with the patient. - Be open to floating to nearby clinics to pick up additional hours if needed. - Wear scrubs as per the dress code requirement.
