About The Position
Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: None Experience: Minimum 1 full year Adult Home Health or Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Proficiency in OASIS - Proficiency in HomeCare HomeBase (HCHB) used within the last 6 months Job Description: - Provide, coordinate, and direct home nursing care in accordance with physicians' orders and agency policies. - Supervise and evaluate the performance of Home Health Aides and Licensed Practical/Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Travel up to 75 miles from the home clinic with reliable transportation, a current driver's license, and liability insurance. - Handle an average of 5-8 patients per day, contributing to 30 points per week depending on the activities and types of visits. - Types of cases may include Start of Care, Wound Care, and Follow-up Visits. - Participate in on-call rotations approximately 2x/month. - Cover the Portland metro territory with approximately 300 miles of travel per week. - Use electronic medical records systems such as HCHB/Point Care. - Ensure all documentation is completed within 24 hours of the appointment with the patient. - No guaranteed hours. Must achieve a weekly productivity point total equivalent to 40 hours per week. - Be open to floating to nearby clinics to pick up additional patients if needed.
