About The Position
**Job Title:** Registered Nurse **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** Not guaranteed **Experience:** Minimum 1 full year Adult Home Health experience or 1 full year Hospice experience **License:** Must have license in hand **Certifications:** CPR, BLS **Must-Have:** - Proficiency in HomeCare HomeBase (HCHB) EMR system and recent use within the last 6 months or completion of a retraining course during compliance - Proficiency in OASIS for RNs, PTs, and OTs **Job Description:** - Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. - Responsible to Clinical Manager. - Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Must possess reliable transportation, current driver's license, and agency-required liability insurance. - Expectation to manage an average of 5-8 patients per day, equating to roughly 6-8 points, with a weekly goal of 30 points. - Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. - Be prepared to travel up to 75 miles from the home clinic; mileage reimbursed at the IRS Standard rate. Travel will be compensated from patient home to patient home only. - Expected to accept every patient on the schedule, regardless of distance, and adhere to a documentation completion deadline within 24 hours of the patient appointment. - Possible participation in weekend and on-call rotation based on location and profession specifics. **Additional Considerations:** - Local candidates are accepted at the same rate. - Must be open to travel and flexible in floating to nearby clinics if patient census is low. - Documentation delays impacting billing and follow-up patient care may result in contract cancellation.
