About The Position
Job Title: Registered Nurse I Job Specialty: Home Health Job Duration: 13 weeks Shift: Day, 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No Experience: Minimum 1 full year in Adult Home Health or Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Proficiency in OASIS and HomeCare HomeBase (HCHB) EMR - Reliable transportation, valid driver's license, and liability insurance - Open to on-call rotations - Ability to travel up to 75 miles from the home clinic Job Description: - Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures. - Competently apply the nursing process. - Responsible to the Clinical Manager. - Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Manage an average of 5-10 patients per day, equating to roughly 6-8 points. - Handle types of cases such as Start of Care, Wound Care, and Follow-up Visits. - Complete all documentation within 24 hours of patient appointments. - Meet productivity expectations of 30 points per week, equivalent to approximately 40 hours per week. - Be willing to float to a nearby clinic to pick up hours if needed. - Cover the Northern Neck territory with travel mileage reimbursed at the IRS standard rate. Additional Notes: - Workload and staffing are driving this job request. - Expectation includes accepting all patients on the schedule regardless of their distance from the previous appointment.
