About The Position
As a Home Health RN, you will be responsible for providing direct patient care in a home setting. Your primary duties include assessing patients' health status, developing and implementing nursing care plans, and collaborating with physicians and other healthcare professionals to deliver comprehensive care. You will also educate patients and their families on disease prevention, treatment options, and self-care techniques. Responsibilities: Patient Assessment: Conduct thorough initial and ongoing assessments of patients' health status, including physical, emotional, and social factors. Care Planning: Develop individualized care plans in collaboration with the patient's physician, outlining nursing interventions, goals, and expected outcomes. Clinical Procedures: Perform various clinical procedures, including wound care, medication administration, intravenous therapy, and patient education. Monitoring and Evaluation: Monitor patients' progress and response to treatment, modifying care plans as needed. Evaluate the effectiveness of interventions and adjust accordingly. Patient Education: Educate patients and their families about their medical conditions, treatment plans, medications, and self-care techniques to promote wellness and independence. Coordination of Care: Coordinate care with other healthcare providers, community resources, and support services to ensure continuity of care for patients transitioning between healthcare settings.
