About The Position

No locals acceptedInitial Vaccination series is required. Exemptions may be approved for religious or medical reasons. Email Scott W @ for exemption form to be included into submittal file.The Interim Director will oversee the operations and staff (approximately 24 staff) for Case Management, Social Services, on-site Spiritual Care services and Utilization Review.Responsibilities include (but are not limited to):Directing the work of the Case Management department in accordance with federal, state and local guidelines and regulationsFacilitates appropriate utilization of hospital and community resources to provide high quality, cost-efficient care, and achieve throughput, length of stay, and clinical quality goalsBuilds and maintains strong working relationships with post acute partners to ensure successful transitions of care and positive patient outcomesReviews re-admissions and oversees opportunities to reduce preventable re-admissions; presents reports and statistics to the Utilization Review/Quality Oversight CommitteeResponsible for the department s budget, contracts with external vendors, and applicable policies/ standard operating procedures/ work instructions; plans and implements recruitment and retention strategies for areas of responsibility in collaboration with Human Resources.Job Submission requirements: Minimum 7-10 years related experience (MUST have Case Management Managerial/Supervisory/Director experience)- RequiredBSN or combination of BSN and MSN - requiredMust have Hospital/Medical Center experienceSubmitting Agency should have an Active NH agency licenseProfile History going back 7 yearsCase Mgmt skills checklist/related competency examSupervisor references from within previous 3 years of employment (2 separate references must be included in submitting profile)License Certificate (AHA) Hampshire Guaranteed Hours: 40 Contract Weeks:91
