About The Position
ID: 52990731 Shift: 8H Days Days Description: Unit : Case Management*Required Certifications: BLS*- Other Certifications/Specialty certifications requirements?: PRI & SCREEN certifications (can be taken online through the School of Albany- typically take a couple hours for each certification; Cost approx. $300/certification) - Role: Expected to have knowledge of discharge planning and making referrals. Will typically be working on a MS or Tele unit and will split the floor with a Social Worker. Must be able to function independently on the unit and working collaboratively with an interdisciplinary team and patients for discharge planning. Expected to round on the unit with physicians/team. Need to have a positive attitude, be able to critically think, and good time management skills. Utilization Review (UR) is a separate dept- UR will do insurance approvals and arranging transportation- Case load: Floors typically have 33-40 patients; Most floors are split with 1 CM & 1 SW; Caseload: 17-25 patients. Pilot Floors will have 1 CM & 2 SWs; Caseload: 12-17 patients. Need to be able to handle at least 20 patients. Typically placed on Med-Surg or Tele Floors following Internal Med and Family Med.- EMR: Allscripts- Do you require InterQual criteria or Milliman experience: Prefer some knowledge of CarePort, MCG, and/or InterQual. Prefer some Utilization Review experience.- Is this position for UR, discharge planning, or both?: Discharge planning - What areas do they cover: Inpatient - Common types of patients & job duties: Discharge to Home, Community, or SNF. Obtaining housing, Shelter Packets for Homelessness, Hemodialysis packers, KCI referrals for wounds vacs, and more. A binder is provided as a resource.- Experience or 'Must have' skills: Must have 1 year of inpatient case management experience. Prefer prior critical care experience- CM Manager/Supervisor: Manager Reiny - Shifts & Scheduling: Shifts M-F 8-4:30 (7.5 Hr shifts with a 1 hr lunch). Breaks are per facility policy. Management is supportive of taking a full lunch break.- Weekend Requirements: Occasionally (rare- with manager approval). Caseload on weekends is different.
