About The Position
Job Title: Registered Nurse Case Manager • Day Shift
Job Description:
Our client is seeking a dedicated Registered Nurse Case Manager to join their team. The ideal candidate will have a strong background in case management and acute inpatient care, with a deep understanding of illness characteristics and treatment expectations. This role requires excellent communication skills and the ability to efficiently utilize resources to promote quality, cost•effective outcomes.
Responsibilities:
• Work a 40•hour week, with shifts from 7:30 am – 5:30 pm. Start times may vary. No major holidays required.
• Provide coverage every 6th weekend as needed.
• Assess, plan, implement, monitor, and evaluate the options and services required to meet a patient's health needs.
• Manage the length of stay and promote efficient utilization of resources.
• Determine medical necessity and appropriateness of admission with InterQual screening and inpatient list from Medicare.
Requirements:
• Minimum of 3 years of experience in case management, with a strong understanding of current MS patterns of practice.
• Knowledge of benefits, plan contracts, eligibility issues, and requirements.
• BSN certification is required.
• Preferred: 5+ years of experience with UM & CM, Epic.
• Preferred: CCM or related certification.
