**Job Title:** RN - Case Manager **Job Specialty:** Case Management **Job Duration:** 13 Weeks **Shift:** 08:00 - 16:30 **Guaranteed Hours:** Not specified **Experience:** Minimum 1 year **License:** RN Licensure **Certifications:** BLS **Must-Have:** - 2 years of Acute Case Management experience **Job Description:** - Provide case management services for both inpatient and observation patients assigned. - Perform a comprehensive assessment of psychosocial, medical, and discharge needs of patients and their families. - Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary team. - Evaluate the progression of care using InterQual throughout the episode of care. - Work collaboratively with RN case managers and the multidisciplinary team. - Perform assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs. - Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care. - Maintain the ability to multitask and prioritize workflow and needs. - Adapt to changes and be a team player. - Business casual dress code is required. - Remain calm in crisis situations and deliver information in a clear, concise manner. - Build trust and act with authenticity. **Additional Information:** Join a team recognized by the administration for maintaining a consistent length-of-stay ratio.
