About The Position
**Job Title:** Registered Nurse (RN) **Job Specialty:** Case Management **Job Duration:** 12 Months **Shift:** Monday - Friday, 8:00 AM - 4:30 PM **Guaranteed Hours:** Not specified **Experience:** Minimum of 2 years of recent hospital case management experience **License:** Active Indiana RN license **Certifications:** BLS certification through the American Heart Association **Must-Have:** - Experience in RN Case Management **Job Description:** - Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians to provide comprehensive care coordination and discharge planning for each patient. - Utilize critical thinking to develop and execute effective discharge planning. - Promote effective and efficient utilization of clinical resources. - Monitor all areas of a patient's stay for effective care coordination and efficient care facilitation. - Conduct medical necessity reviews for appropriate utilization of services from admission through discharge. - Document all patient review records according to regulatory, legal, and coordination requirements. - Perform reviews for third-party payors as appropriate. - Stay current regarding reimbursement modalities, community resources, and case management or psychosocial legal issues that affect patients and providers of care. - Conduct comprehensive, ongoing biopsychosocial assessments of patients and family to provide timely and safe discharge planning. - Refer cases where patients and/or family would benefit from additional resource support to the Social Worker, where applicable. - Serve as a patient advocate and enhance collaborative relationships to maximize the patient's and family’s ability to make informed decisions. - Participate in interdisciplinary patient care rounds and/or conferences to review treatment goals, optimize resource utilization, provide family education, and identify post-hospital needs. - Collaborate with clinical staff in the development and execution of the plan of care, and achievement of goals. - Demonstrate knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care to the patient population served. - Coordinate the provision of social services to patients, families, and significant others to help them deal with the impact of illness on individual family functioning and achieve maximum benefits from healthcare services. - Initiate patient and family education and discharge planning at admission. - Arrange for discharge and post-hospital care of patients through institutions and agencies within the community.
