Job Description:

• Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines.
• Ensure accurate and timely electronic submission of all MDS assessments.
• Review and interpret resident medical records to assess and document care plans.
• Collaborate with interdisciplinary teams to ensure optimal patient care.
• Provide ongoing education and training for staff on MDS processes and requirements.

Education + Certification Requirements:

• Valid RN license in the state of Minnesota.
• BLS certification; ACLS preferred.
• LTC experience preferred
