Shift: M-F Days
Ratios: 1:5
2 Years Minimum Experience Required
Must have 1 prior travel assignment
Weekend REQ: rotating weekend if hired for perm
Certs REQ: LCSW, MD STATE LICENSE
Locals Accepted: Local Rate within 60 Miles - See AM
Skills Req: Prior medical/clinical social worker experience ; Provides clinical social work services to identified patients and families. This includes outpatient and inpatient services. Includes individual family and group counseling regarding the impact of illness and hospitalization.
Functions as a coordinating link between patient, family, hospital, and community resources to assure continuity of care. Palliative Care Skills and experience Required.
Skills Preferred: Care coordination, Assess response to treatment, Collect and analyze patient information, Develop plan of care to meet patient/caregiver needs, Establish and revise goals, Review of Health history, Collaborative care, Coordinates quality care in cost effective manner, Documents pertinent information to facilitate care coordination, Advance Directives, Knowledge of cultural issues, Participate in development of treatment plan, Provide patient and family education, support and advocacy, Work with multi-disciplinary team to ensure continuity of care
