Description: Connected Health Care is seeking a dedicated and dynamic Contract Physical Therapist to join our client's outpatient clinic in Espanola, NM. Job Responsibilities:

• Provide exceptional physical therapy services to patients in an outpatient clinic setting.
• Develop and implement individualized treatment plans to address patient needs and goals.
• Collaborate with interdisciplinary team members to optimize patient outcomes.
• Maintain accurate and timely documentation of patient progress and treatment interventions.

Requirements:

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active state licensure as a Physical Therapist.
• General certifications as required by the state.
• Additional certifications may be required based on client preferences.
