Connected Health Care is partnering with a leading healthcare facility in San Jose, California, to find a dynamic and skilled Travel Physical Therapist for a 13-week contract. Job Description:

• Position: Travel Physical Therapist
• Location: San Jose, CA
• Contract Duration: 13 weeks
• Shift: Day, 8 hours/day, 40hrs/week
• Start Date: April 8th, 2024

Key Responsibilities:

• Conduct patient evaluations and develop individualized treatment plans.
• Provide hands-on therapy to patients and educate them on proper exercise techniques.
• Collaborate with a multidisciplinary team to ensure comprehensive patient care.
• Maintain accurate patient records and documentation.
• Stay up-to-date with industry best practices and trends.

Education and Certification Requirements:

• Master's or Doctorate degree in Physical Therapy.
• Current California state licensure as a Physical Therapist.
• BLS (Basic Life Support) certification.
• Minimum of 2 years of clinical experience.
