About this Role
Belle is hiring remote Physician Assistants, Clinical Nurse Specialists or Nurse Practitioners that are licensed in the state of Florida. The primary purpose of the role is to conduct telehealth Annual Wellness Assessments and establish care plans for qualifying, polychronic members. This role is 100% remote, allowing you to utilize your healthcare expertise while maintaining a flexible working environment. In this position, you will perform wellness evaluations, gather comprehensive patient histories, and collaborate with our team to enhance patients’ overall health. Anticipated availability is 9am-5pm EST, Monday-Friday.
Responsibilities
- Conduct telephonic annual wellness visits.
- Identify patient needs, provide health education, and offer lifestyle recommendations to support preventive health measures.
- Create patient care plans.
- Document patient interactions and maintain accurate records in compliance with company policies and healthcare regulations.
- Collaborate with the care team to ensure patients receive appropriate referrals and care.
- Identify and enroll patients who would benefit from Chronic Care Management (CCM)
- Review ongoing CCM patient care plans.
Experience, Skills, & Personality
- Appropriate CNS, NP, PA licensure/certification in the state of Florida.
- Compliant with state of Florida requirements for CEUs and licensure
- Experience in telehealth, wellness visits, and preventative care is required.
- Excellent communication and interpersonal skills, especially in a telephonic setting.
- Strong organizational skills and attention to detail for effective patient documentation.
- Comfortable utilizing and managing electronic health record (EHR) systems, medical software/technologies to ensure accurate documentation, care coordination, and data security.
