About the Job
The PACE Meeting Coordinator assists the Center Manager in executing daily Interdisciplinary Team meetings. Additionally, this role is tasked with successfully and efficiently leading the team meetings to ensure documentation meets regulatory requirements and assisting in guiding discussions to keep the team members on track.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Works with the Center Manager to update the daily meeting agenda; this includes monitoring email communications related to participant care issues or changes and sending the agenda to the teams daily.
- Leads the Interdisciplinary Team (IDT) meetings, ensuring that discussions are held efficiently and respectfully.
- Keeps a detailed record of pertinent information during the meetings to ensure all discussions and recommendations are captured in the Electronic Medical Record (EMR) system and maintain compliance with regulatory requirements.
- Sends out IDT follow-up communication daily.
- Assists with the auditing of IDT documentation, including, care plan signatures, Service Delivery Request (SDR) signatures, care plan intervention updates, and signed notes and discharge summaries.
- All other duties as assigned
Qualifications (Education and/or Experience)
To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Bachelor’s degree in health administration, communications, or related field; or a combination of equivalent experience.
- Ability to eloquently write and communicate.
- At least one year of experience working with a frail or elderly population is required.
- Experience in a PACE program is preferred.
- Strong organizational skills.
