Job Description
The Ambulatory Care Pharmacist participates with other team members in the primary care setting to manage our patient's health. He/she is primarily responsible for developing and implementing pharmacy related processes and procedures within the Christ Hospital Medical Associates that support improved health outcomes for defined populations. He/she will assist providers in developing effective medication regimens, provide assistance for patients with polypharmacy issues and review patients with high risk medication profiles. They will work with our payer partners, quality team and care management team to develop processes to improve medication adherence and support value based contract initiatives.
This position is focused in oncology and primarily supports our outpatient oncology practices and their oral oncology counseling program.
Responsibilities
Complex Medication Management
Comprehensive Medication Reviews (CMR) and Targeted Medication Review (TMR)
• Development and provision of a Medication Therapy Management (MTM) program
• Performs comprehensive medication reviews on defined populations. Identifies potential therapy changes that could improve the management of chronic illness and decrease the cost to the patients.
• Prospectively recommend and evaluate individualized drug therapy
• Assist in the design and development of process: Identification of patient populations, review process, communication plan with providers, documentation process and metric development.
• Identify patients on high risk medications and collaborate with providers to discuss options.
• Direct collaboration with hospital based, community and payer employed pharmacists
• Work with patients to solve problems with their medications and improve adherence.
• Using motivational interviewing to address patient's concerns or resistance to taking medications
Transitional Care Management
• Assist the care managers with patients in the TCM process that have significant changes to previous medication regimens and /or medication discrepancies.
• Work with patients to ensure understanding of medication plan, special instructions and identify barriers to taking prescribed medications.
Collaborate with any provider to clarify medication therapy plans.
Safety
• Review data to identify patients on high risk medications and work with providers to develop safe medication regimens
• Optimize health outcomes by ensuring safe and effective use of medications
• Collaborate with inpatient pharmacy, quality department and purchasing on medication safety issues and medication recalls
• Prospectively identify potential adverse drug reactions, drug-drug, drug-disease and drug-food interactions, and incompatibilities affecting patient outcomes.

KNOWLEDGE AND SKILLS :

EDUCATION: Doctor of Pharmacy (Pharm D)
YEARS OF EXPERIENCE:
Significant clinical experience with completion of a PGY2 residency in ambulatory practice/oncology required or equivalent experience
REQUIRED SKILLS AND KNOWLEDGE:
Ability to manage and prioritize multiple tasks, knowledge of Excel, Word, Visio, Outlook an PowerPoint and the ability to learn other computer skills. Must have good organizational skills and work professionally with doctors, administration, staff and patients. Strong communication and interpersonal skills. Candidate should possess leadership skills and be self- motivated.

Licensed to practice pharmacy in the State of Ohio
