POSITION DESCRIPTION
Job Description: Supporting Housing Case Manager/Counselor
Position Summary: The Permanent Supportive Housing Case Manager is primarily responsible for assisting individuals obtain and remain in permanent housing and mover towards self-sufficiency. Case Managers will network, collaborate and coordinate housing, behavioral health, substance and mental health services, community services and educational/employment services for participants in the program. This Case Manager also conducts individual and group sessions, documents individual participation, and submits required reporting data.
Education Requirements: CASAC, Bachelor’s or Master’s degree in health and human services. Case Management and familiarity with working with individuals with co-occurring disorders is preferred.
Experience Requirements: Experience working with individuals with substance abuse and/or mental illness, homelessness, familiarity with community agencies and entitlement programs, flexible scheduling during the week.
Principal Duties and Responsibilities:
- Perform housing intake assessments to determine the needs of the participant
- Assist individuals in securing safe, affordable housing, develop relations with landlords and community relationships
- Assist with locating and securing safe, affordable housing
- Perform housing inspections, establish rental agreements and visits to ensure apartments are properly maintained
- Develop relationships with landlords
- Assist participant in development of recovery plans
- Provide linkage to entitlement programs, medical, mental health and substance abuse treatment
- Provide case coordination and collaboration with treatment programs and other community resources
- Assist and support participant’s progress working with the employment specialist, training programs and employers to achieve education/vocational goals
- Monitor participant’s progress through regular home and office visits
- Utilize harm reduction interventions and motivational strategies to provide client centered services that keep them in housing
- Provide direct care services including individual and group meetings, community meetings, and assistance with entitlement programs
- Maintain individual case records, collect data and reports as required
- Must have a valid driver license
- Must be available to work flexible hours, including on-call availability
